TOC is positioning page numbers differently for Headings of same style

Here's an example of what I mean. All entrees in this TOC are same level (1). but the TOC is treating them differently

FIRST CHAPER 1
SECOND CHAPTER 2
THIRD CHAPTER 3
FOURTH CHAPTER ...........................................4
FIFTH CHAPTER ..............................................5

Any ideas?
