Default WP Document as Sample

Paul,

I have saved a WP document as a Doc and hopefully it will show what I am talking about.
In the document I colored the areas red that I fill in through dialog boxes that then puts the info in the correct place.
I'm not good for explaining what I am trying to do, but I appreciate your help and will attempt to follow all directions.
