Default How to make tabbed fields?

I need to create a form that only needs to be filled out in certain parts. See the attached example of a similar form I already use. The grey areas are the only parts I can change. I just tab through the different fields and change what I need to and re-save it with the new info. I don't know how to create a new document with the same tabbed fields, though. Any help is much appreciated!
