Default PDF Insert

We use subcontractors for an annual event (approx. 300). Every year I do a mail merge from an Excel list that is 3 pages for them to complete and return. I then print the standard W-9 form and add it to each set. I would love to insert it directly into the mail merge but the resolution is horrible. Any thoughts?

You can mailmerge with your PDF as an attachment using http://www.gmayor.com/ManyToOne.htm in one to one mode, but without seeing what you are actually merging, it is difficult to determine what can be done with the message body.
