Table of contents removes 'levels' when 'editing'

Thanks all for your suggestions.
I tried only using 'Level 2' for Heading 3 and Appendix but once i went into edit again - they were removed and deleted.
Appears my "show levels" (in Table of Contents/General) is set at '2' and when i change it, to 3 or 5, it defaults back to 2.
