Are the levels part of the style definition too? Make sure they are. When you start a TOC and choose to have 5 levels, it automatically (I think) picks all styles that have any of those levels as part of their definition. You can add or subtract styles from the TOC in the TOC menu, but to have them show up automatically I think you have to the levels be part of the style definition.
