This has nothing to do with headers/footers. I understand your confusion, though.

You want to restrict editing. I find it easiest to do this using Protect Document on the Developer Tab but you can do it on the Review tab. The thing is that you probably want to include Content Controls for the variable information you want to type and allow editing of those for everyone.
