Unhappy Table of Figures repeats caption references.

I have a document that I have inserted a dozen figures and have inserted captions on each. As I have edited and re-edited the document, sometimes I have had to move the figures around to fit on the page properly. I inserted a Table of Figures (TOF) immediately following the Table of Contents. Each time I edit the document, and update the TOF, the caption for a given figure is duplicated in the TOF and an additional figure number is inserted. The reference code on each entry is slightly different, but there is no visible duplication in the text of the document. Also, the TOF is mixing up the proper placement of each entry. I can't seem to get the TOF to cease referring to a caption that doesn't seem to exist . I have included a screen shot of part of the TOF that is getting created that shows some of the references as they display and some that I toggled the field code so you can see what might be going on behind the scenes. Does anyone have any suggestions.
