If the uploaded file remains in the doc/docx format, there is no reason the layout would change. If you're converting to another format, though, you're at the mercy of that format's image handling capabilities. Do note, too, that even with the doc/docx formats, the layout in Word is liable to change if the document is viewed on a system using a different printer driver than the one used on the PC/Mac used to create the document. If you want to guarantee your layouts remain unchanged, use the PDF format.
