Hi everyone! Can't figure out how to create a template ( please see the picture)

Picture doesn't help much, especially sideways. Still don't know what you want. Two blank boxes at the top of the page? Do you want a border around the page?

Do you want to type in the boxes at the top or are they for decoration? Do you want every page to look like this?

Ummm. Yes. The picture is kind of useless. It does not tell us anything. Try actually describing what you want. Be specific. Start by answering the questions Charles asked.
