You could perhaps try creative use of tabs, indents and SEQ fields … so each block would consist of

the manually-entered 'score', followed by a tab
a SEQ field for reliable auto-numbering, followed by a tab
the text of the question, with the style set so that the second and subsequent lines wrap neatly
