I had similar problems as Alpina.
Shauna Kelly's method is correct, but it's easy to make a mistake when you are including the numbers from the previous levels. I found you have to be extremely careful and methodical and get the sequence right.
After a few frustrating attempts I deleted all the heading styles and slowly but carefully started from scratch, following Shauna's instructions. It worked, but it took time.
