How do I insert an Enhanced Screentip?

I want to insert an enhanced screentip which has 5 lines in Word 2010 and Word 2003 docs.


I can do the 1 line screen tip OK but don't see a command for Enhanced Screen Tip
From what I have read it seems that you can read a Microsoft installed multi-line message or enhanced screentip on a toolbar or ribbon but you cannot insert the same into a word doc that you create.
I have a Word Doc with locked form fields and need to insert a hover comment that is about 5 lines that explains how to unlock the form so the user can past a picture and then lock the form.
Any help would be appreciated
