I'm not sure what the cause is, either. However, the fix is quite simple: copy the word 'Cross' and its index entry from page 5, then go to page 6 and replace each of its 'Cross' words and their index entries with the one from page 5. Then press Ctrl-A, F9 to update all references in the document.
