How did you mark the Index entries?
If I create some text with 'Angel', 'angel' and 'Los Angeles' and tell the Indexer to mark all 'Angel' or 'angel' entries, 'Los Angeles' does not get marked. The reverse is also true. Indeed, telling the Indexer to mark all 'Angel' entries doesn't mark any 'angel' entries, or vice-versa.

I indexed manually; maybe that's the problem? I went through the document, highlighted a word I wanted to index and when it showed up in the "Mark Index Entry" I hit "Mark." I did it this way because I wasn't sure of all the words I wanted to index so I didn't think automatically indexing would work well.

Also Cross and cross are different entries, which also include crisscross and across.
