I can't reproduce the unlabelled header/footer boxes. Are these documents based on different templates? Do all documents based on the same template give you the same display?
Unable to paginate? How are you trying to paginate? Try clicking in one of the boxes and inserting a page number. See How to set up a document with front matter numbered separately Sections / Headers and Footers in Microsoft Word 97-2003
