Default Can't properly number these damn headers

I have a 23 page report that is taking me longer to number properly than it did write.

Here's what I need,

Pages 1 & 2 need no header
Pages 3,4 & 5 need to numbered Last name III, IV, V
Pages 6-21 need to be numbered 1-15
Pages 22 & 23 should also have no header.

The format of the numbering is Lastname 1, Lastname 2, aligned in the right hand corner.

Anyone mind helping me out with this? Thanks.

You need section breaks. You want the headers between the sections to not be linked to previous.
