You may have a printer issue if you have set your parameters correctly.
Because MSWord does not provide the parameters you specify...you can create the labels yourself at
Mailings/Labels/Options/New Label and defining the parameters. I have attached a file with your parameters as an example (I used a "Table" to create. You may opt to create with another method).
