20+ years ago I wrote a one-click Indexer for MSWord in Word6.0, and have since migrated it to Word2003/VBA. I use it every week, but used it extensively when I was paid a pittance to develop training courses and manuals. It really is just ONE Click, and it is fully customizable by you, the user, through an MSWord table holding simple rules. Also it is FREE.
