For me, with Word 2010 both versions align the callout with the paragraph the selection was in.

Hi again sorry not right

No neither are working correctly as not only do they produce a callout at the top of the page instead of with the paragraph it comes from, they also duplicate the selected text within the paragraph as well.

What happens when you use Word 2003?

I suspect there's something wrong with either your current Word session, or with Word itself. Try closing & re-starting Word. If that doesn't resolve the matter, try repairing Word (Word Options|Resources|Diagnose).
