Please forgive me if this isn't in the right place but what I'm trying to do (with no experience whatsoever

) is create a word document that is a fillable form. It would have customer detail text fields i.e. name, address, telephone, etc. It would have a large text field that contains scope of work details and check boxes that pertain to various contract exlusions.
Essentially I what the form to be filled and a new document or page to be created that shows those details that were entered in addition to the text that would be related to any checkboxes they'd selected. Of course I'd format the new document to show static information.
Is this something that is beyond word or is it something that's doable?
