Sorry for the delay reply.

1. The format can be docx. Not an problem.

2. The populating fields were all the text boxes that are in first page, till before start of sl. No.1. boxes.In other words, except check boxes and optional buttons in Page 1 & 2. Third page not of use.

Those needs to be clicked for the desired result of the staff.on 4th and 5th pages, the total of all the selected check boxes and Option buttons has to sum up and get displayed.
