You are asking questions that reveal this is a learning experience for you. The thing you are trying to achieve is not entry level and you are attempting something well beyond your current knowledge levels.

I can help you learn but I'm not prepared to create a working example in Word from scratch because it is apparent your knowledge has considerable gaps and I would need to explain more than I particularly care to do.
