I am just about to go into hospital so I don't have time to mess with this now. The code I posted was for a completely different format and will not work with this revised specification.
If the revised spec is mixed with the old spec in the same document then it is a much more major task to work out a solution. I will be out of hospital next week. If you want to contact me privately via my web site after that, we can discuss a price for delivering a complete solution.
