I need some help from you.

I am creating a form in word and in four fields of this form the user will need to insert four images, that is, one image in each field of the form.

I'm trying through the ActiveX control (Image) to do this, I would like a help or a tutorial, as at the time I click on (image) direct the user to the My Documents folder and that this image when inserted into the form is compacted.

Is there somewhere to download a template?
