Smile Word Form/ActiveX drop down menu/value in a different field

I've created a form and want to auto-populate a field with a values assigned in an adjacent ActiveX drop-down menu. I have inserted a bookmark in my menu so it is named and thus may be referenced. As info, I've created all this in a table.

Does anyone know a formula I can use to have the value display in my separate field? Or another way?
