Macro help

I need help with a macro.

This is what I have in my document:

STUDY: CHEST
NAME: BOZO, CLOWN
DATE: 02/27/2012
ID: 123456
DOCTOR: SMITH
FACILITY: ADC

This is what I need that to change that to:

###CHEST,BOZO,CLOWN,02/27/2012,123456,SMITH,ADC

Here's the catch...Those fields change with every report so the name could be longer or shorter, etc.
