Hi Craig,
It appears your document is using ActiveX checkboxes. One thing you need to know about them is that they can't be hidden, so you'd need to use vba to add/delete the additional checkbox controls as required. Since vba would be required whichever way you go, a better choice would be a userform, on which you can hide and/or disable various options. They can also have multiple 'pages', which may suit your needs. On the whole, though, if you need to store this information, and possibly retrieve it at a later date, the best approach might be to develope a database with which to maintain it.
