Default How to Add "Continued" to Table Caption after First Page

This solution is quite simple and I believe the main reason for "Continuous Section Breaks." Basically, put the table between Continuous Section Breaks, de-link the headers, and put the caption (or a bookmark to the header), with "(Continued)" added onto the end, into the header of the section containing the table. With the table's header set to repeat, the 2nd and following pages of the table will look like the first except it will have "Continued" in the caption.
