There are some issues that you might want to consider here. Presumably this 'form' is something that you propose to send out via e-mail for others to complete.
You have no control over how your message will be viewed by the recipient, so any element you add to a message is likely to be incompatible with the receipient's mail software. Who in their right mind is going to send you credit card information in a format as insecure as e-mail?
