Email with Hyperlink and a Read Button added

I need a way to create a E Mail with a hyperlink to a document (no problem) but I also then need a button that the reader can click on this will send a read and understood response.

We basically need to send all our user a document that they need to comfirm that they have read and understood.

Any ideas really appreciated!
