Default custom contacts form won't transfer to new computer


I transferred all files and folders via transfer cable from my old computer to my new one. The custom contacts form that I was using on my old computer did not transfer to the new computer. The old computer is windowsv7. the new one is windows 8
BTW, before doing the transfer, I synchronized all of my contacts from the cloud into the default contacts folder of my new computer. all the contacts are now in the proper contacts folder but in the default form. But where is my custom form. I clicked on the folder properties and it is not there. Do I have to design that custom form and publish it again on the new computer? thanks for any suggestions. Bob.
