I'd suggest using MS Word for this, with text formfields for things like name & date input and dropdown formfields for answer selections. See, for example: https://www.msofficeforums.com/word/...s-answers.html

While it's easy enough to have the selection from a dropdown trigger the display of a correct/incorrect response, that seems to defeat the purpose, as an ignoramus can simply go through each answer until a 'correct' response is achieved without ever learning anything. A more sophisticated approach would be to display an overall score, only when all the questions have been answered. If it's less than 100%, the user then has to figure out which question was answered wrongly and change the answer.

All the above can be done without macros. If you want to prevent the document being saved unless all the questions have been answered (correctly or otherwise), that would require a macro and the user's cooperation in allowing it to run. If the user doesn't allow the macro to run, they can do as they please.
