That's as simple as:
{IF{MERGEFIELD Bonus Amount}<> 0 "{MERGEFIELD Annual Salary}¶
{MERGEFIELD Bonus Rate}¶
{MERGEFIELD Bonus Amount}"}
where the ¶ is a paragraph break or manual line break.
