Your syntax is wrong - change to
{IF {MERGEFIELD PriorityScore } >= 15 "Very High" "{IF {MERGEFIELD PriorityScore } >= 10 "High" "{IF {MERGEFIELD PriorityScore } >= 5 "Medium" "{IF {MERGEFIELD PriorityScore } >= 1 "Low"}"}"}"} which does work.
