The problem you're having is probably a result of the way the IF field gets evaluated. Try:
{={=MOD({PAGE},2) \# 0}+{IF{MERGEFIELD RunTotal}= "Continued" 0 1} \# ";;'{QUOTE 12}Intentionally blank page'"}
