Provided there are columns in the data source for each additional item, you can use mergefields to reference each of those columns - only those with data will be populated.
Alternatively, if there is a column in the data source with a code that can be used to identify what additional items a given recipient gets, you can use IF fields to conditionally output the items corresponding with each code.
