Have you experimented with any of the tutorial's fields, simply copying them to the sample mailmerge main document to see how they work? The field structure in your document isn't anything like what the tutorial uses. Besides which, I don't know how you intend your output to appear.

PS: The fields from the MS website do work; they just don't do so as well as the fields I've crafted.
