Create a two line header
Is it possible in Excel to create this 2-line header? It was formerly a word doc. I got everything converted to Excel with the exception of the header -- please see attached gif.

What do you mean by 2-line header? A print page header, or defined Table headers, or header row for classic Excel table?

A printed document then?
From Excel Menu: Page Layout > Print Titles, "Rows to repeat at top" enter row numbers like $1:$2, or select them from picker.
