Pecoflyer has a reputation beyond repute

As the lookup values now are identical, the LEFT function can be dropped
=INDEX($B$4:$AF$7,MATCH($C11,$A$4:$A$7,0),MATCH($C 12,$B$3:$AF$3,1))
