Default How can match or lookup values from two separate tables?

On the attached worksheet, what is a good formula for taking the personnel # and Occur Date on tab entitled "Report 1" to match the Disciplin.Event Date On tab entitled "Report 2" and bring back the "disciplinary event" to "report 1" worksheet.

Note that there are 5 lines of information on Report 1 and only 4 lines of info on Report 2. Dates must match.
