It turned out that I had *somehow* changed the Ctrl-B and Ctrl-I keys to Ctrl-Shift-B and Ctrl-Shift-I !! Bizarre and I don't remember doing it, but I must have! Now reset to what they should be.

Strange how they worked for turning the toggle ON but not OFF though!
