In that document, the content controls populate a series of custom document properties, named Sbjct, Stmnt, Dsclr & MjTpc, respectively. Those have to be created in the document beforehand, via File>Properties>Advanced>Custom. Then, for cross-referencing purposes, DOCPROPERTY fields are used, each one referencing its own custom property.

There is another way, though, that requires no code, custom document properties or DOCPROPERTY fields: simply copy a given content control and use Paste Special>Paste Link to replicate the data.
