Click in one of the bogus entries in the body of the document. What is the style? It may look like Normal in its formatting but be in H1 or H2.

In Word for Windows, the Document Map (not the Navigation Pane) was a flawed feature that caused problems when used for editing rather than navigation. I do not know whether the Mac version has the flawed feature or not.
