Question survey box??

i'm working on a survey type document in word (like multiple choice). does anyone know how to insert a box where someone could mark an X for the most appropriate answer? i tried a text box but couldnt make them very small and the alignment was off. i'm using office 2007. thank you very much for your help

Have a look on the Developer Tab and then Content Controls, there is an icon with a little tick in it which you can check/uncheck as necessary. You can copy this and paste it next the to appropriate items, they even work in tables.

If you cannot see the Developer Tab, right anywhere in the ribbon and choose customize, the right hand panel shows the Tabs available, check the Developer Tab and close.
