Default Custom Tab on the ribbon missing

I have created a .docm file in Word 2010 complete with macros and an additional tab, groups and buttons on the ribbon. The ribbon additions were created using the Custom UI Editor. The reason for doing this is so the document can sit in SharePoint and people in our company can use it as a sort of de facto template. This means they can use it anywhere in the world and don't need access to an original template to keep using the macros, etc.
My problem is that on one occasion (so far) when a user has downloaded a copy of the file and saved it as a .docm (to retain the macro functionality) the tab on the ribbon disappeared.
I have checked using the Custom UI Ed. and the XML code is gone.

So, a) is using a .docm in this way OK (it seems to work fine except for this one occasion) and b) is this a just a one off, or is there something else going on here (dare I use the b word, bug)
