Default Help with template fields

I am new to this forum and have question about MS word 2007 templates.
I have existing template with individual fields that pop up on launch of template.



I want to have all the fields pop up in one window instead of pop up individually.

Could someone please show me how to do this. I have attached the .dot file in a zip.

Hi Sue,

Your document uses a series of FILLIN fields. By design, these only give discrete prompts. What you're describing could be served via a userform, for which some programming in vba would be required.

Are there any userform that contain vba programming that I could look at?
A sample form or template?
