Default Form submenu not consistent!

When I set up a form in Word 2004 on mac or 2003 on pc, I experience the same problem when I enter a two digit number in a field.

If I set it up for a two digit number and enter "0" or nothing in the style field I get inconsistent results if the leading number is "0" as in 01 or the end number 10 which on a different line will give me "1"!

How can I restore the leading or trailing 0? One line of fields; eg - the first will be normal or not and the others vary with the same preferences! Where is the control for this?

I need to get the form operational for a medical office. Any help will be appreciated!
Thanks.
Dr. Joe

Default Addendum:

I tried another number property which also didn't work!
I changed to text instead which helps me get the work out but does NOT solve the problem. Anyone have the solution?!

Please describe the technique you are using to make the form. For example, are you using a text from field from the Forms toolbox, or are you using the Controls toolbar, or what?

Cool forms submenu...

I am entering the field from the forms toolbox, then selecting properties, selecting "number" or "text" (which is the only one that works) and selecting 2 characters and a number style (if used with the number selection).
It works when I use character but not numerical types.
What do you think?
I've done this before on someone else's machine and I never had a problem with this!

Default Addendum 2...

I also tried a third number format and it did the same thing; it took off the 0 at the end of 10! I scoured MSWord for options or customize under Tools but came up with no way of altering this activity or default.

Default Addendum 3...

I just realized that the form was made by placing a picture behind a table and then turning off the lines, so actually we're dealing with a form entry within a table. That's probably causing the inconsistency!
Is there a way to affect this situation using some sort of table control, command or preference?
