I doubt that this can be done but I'm going to see if any of you can figure this out. In the attached word example I have created a format that I want to use as a template that acts like a header or footer. So when you start a document with this template it is greyed out and cannot be changed just like a header or footer. can this be done and if so how?

I have not looked at your sample. You can tell me if I need to.
Put your text/images in the header(s) and footer(s) in a document.
Go into them for editing and select them.
On the Developer Tab apply the Group Content Control.
Save the document as a Word template (.dotx).
Use that template for starting new documents.

There is another solution that requires a macro.

You already have the answer, put it in your header. The table you put on the left side of the page can be cut and pasted into the header (you already have that with text wrapping turned on).

@guessed - I've tried this approach and the problem is that the header (or footer) height changes, which create problems when trying to type in the main body of the page. the main body becomes very small and will fit only a few lines before it starts the next page.

@Charles - putting this in the header worked, but when I put it in the footer the footer height increases creating a very small writing space. thanks so much for the help.

If you put all the text in the header you get more than "only a few lines"
