I have not looked at your sample. You can tell me if I need to.
Put your text/images in the header(s) and footer(s) in a document.
Go into them for editing and select them.
On the Developer Tab apply the Group Content Control.
Save the document as a Word template (.dotx).
Use that template for starting new documents.

There is another solution that requires a macro.
